Onsite Mobile Lab Services LLC is a mobile specimen collection service serving Southwest Florida. We provide professional mobile phlebotomy, specimen collection, specialty kit collection, wellness testing support, DNA collection services, and other laboratory support services as permitted by applicable laws and regulations.

Onsite Mobile Lab Services LLC maintains a CLIA Certificate of Waiver and operates in accordance with applicable federal and Florida laws, regulations, patient privacy requirements, infection control standards, and specimen handling procedures.

Specimens collected during appointments may be delivered, transported, or shipped to Quest Diagnostics, Labcorp, specialty laboratories, research organizations, healthcare facilities, or other authorized testing facilities as directed by the ordering healthcare provider, testing company, or patient service requested.

Onsite Mobile Lab Services LLC does not diagnose medical conditions, prescribe treatment, or provide medical advice. Patients should consult their healthcare provider regarding medical decisions, laboratory testing recommendations, and interpretation of results.

While every reasonable effort is made to ensure proper specimen collection, identification, handling, storage, transportation, and delivery, Onsite Mobile Lab Services LLC cannot guarantee laboratory processing times, insurance coverage determinations, result turnaround times, or actions taken by third-party laboratories, testing companies, shipping carriers, or healthcare providers.

If a specimen is lost, rejected, damaged, insufficient, or requires recollection due to laboratory-related circumstances beyond the patient’s control, Onsite Mobile Lab Services LLC may provide one complimentary recollection visit within the normal service area. Because travel, supplies, collection services, and specimen transportation have already been provided, mobile service fees are non-refundable once services have been rendered.

Onsite Mobile Lab Services LLC maintains patient privacy practices consistent with HIPAA and implements safeguards designed to protect patient information.
